Athletic Training Forms

The links below provide mandatory information that needs to be filled out and sent back to Fort Hays State University.

Returning Athletes
Must complete Parents Information Form, Acknowledgment of Insurance Form, and Physical Update Form.

New Athletes
Must complete Pre-Participation Form, Parents Information Form, and Acknowledgment of Insurance Form.

Sickle Cell Testing
Effective August 1, 2022, all incoming NCAA Division II student-athletes who are beginning their initial season of eligibility with FHSU and students who are trying out for a team, as part of their required medical examination or evaluation, must be tested for sickle cell trait (SCT) and/or provide the institution with proof of a prior test. -- NCAA ByLaw 17.1.5.1 Sickle Cell Solubility Test.

If test results can not be located, a new Sickle Cell Solubility Test will need to be performed to determine the student athlete’s hemoglobin makeup.  This test is blood-draw required and could take 3-5 days for results depending on the lab.

We have the availability to send student athletes to the student health center on campus to get a sickle cell test completed upon arrival.

Results may be forwarded to the team’s Athletic Trainer with the student-athlete’s completed physical.

Policies and Procedures



Pre-Participation Form

Respond to all questions on the form and explain all "yes" responses. Please use your permanent address. Sign and date the form at the bottom. Do Not Fill In The Date Of Exam!

Physical Update Form

This form should only be used by returning athletes who have been participating in athletics at FHSU. Respond to questions as complete as possible and return for review by the athletic training staff.
